Population
Served
- Adults age 18 and older recovering from recent
illness, injury, or surgery
- Frail or socially isolated adults living alone or with
a caregiver
- Adults experiencing a decrease in physical, mental,
memory or social functioning
- Adults who may have any of the following:
Stroke, Parkinson's, Multiple Sclerosis, Alzheimer's, Diabetes, etc.
Our comprehensive program is
designed to provide interventions and services to help delay the
progression of the participant's existing disability and maintain or
improve the individual's health, level of functioning and independence
to their optional physical, emotional and cultural levels.
Attending enables participant's to continue living in their own home
and community while therapeutic activities and holistic care enhances
their quality of life. We become a partner in the provision care
and support for the participant, family and physician.
Program
/ Services Include
- Individual plan of care based on assessment of needs
and strengths provided in a safe and secure environment
- Assistance with activities of daily living
and personal care needs.
- Medication supervision by a Registered
Nurse, LPN or Qualified Medical Assistant (QMA)
- Noon meal and nutritious snacks
- Health monitoring and level of functioning
assessments
- Planned Therapeutic activities such as:
pet therapy, art/crafts, ceramic, table games, daily exercise, walks
and physical activities including indoor volleyball, outdoor activities
in enclosed courtyard, manicures, entertainment, parties and special
events, inter generational programs, community van outings
- Health education and self help tips to
improve level of functioning and interdependence
- Spiritual programs
- Community information and referrals to
support groups/caregiver education and transportation
Additional Services
- Weekly or monthly B/P and weight
- Blood sugar monitoring
Supportive nursing and personal care services are available
for a nominal fee:
- Showers
- Shaves
- Beauty / Barber service
- Massages
Attendance
We strongly suggest a planned attendance,
consisting of at least two half-day units per week to ensure the
participant feels a part of the milieu.
- Participants may attend full days (8 a.m. - 5 p.m.)
- Or in half day increments (8 a.m. - 1 p.m. or 12 noon
- 5 p.m.)
Early and Late hours are 7
a.m. and 6 p.m.
Hours
Monday through Friday
(excluding major holidays) from 7:00 a.m. - 6:00 p.m.
Fees
Unit fees are based on hours
used and families may choose to be billed monthly or bi-monthly.
The facility is surveyed
annually to qualify as a provider for the following programs:
- C.H.O.I.C.E. (Community and Home Options to
Institutionalized Care for the Elderly/Disabled)
- Indiana Family and Social Services Administration
(Bureau of Developmental Disabilities)
- Medicaid Waiver
- Real Services, Inc. (Area II Agency on Aging)
- Family Caregiver Program, Priority Waiver, SSBG
- The Veteran's Administration
- Long Term Care Insurance
Professional
Staff
Quality and Services are provided with a minimum of 1:6 or
1:8 staff to participant ratio. All staff and consultants are
certified and/or licensed, cross-trained professionals.
