The
following is provided for the fair and informed admission to the Milton
Adult Day Services:
It is our intent to serve older persons in the St. Joseph County area
who are living with cognitive or physical impairments without regard to
race, religion, ethnic background or sexual preference These
persons
may be rehabilitating from a stroke, have early to middle stage
dementia, such as Alzheimer's disease or a related disorder, be frail
and need attention or socialization, or cannot safely be left alone.
There may be certain conditions that are inappropriate for the milieu
of this center. These may include but are not limited to some
psychiatric illnesses, traumatic brain injuries, or those who are
profoundly developmentally disabled.
Upon assessment and history of the participant, the enrollment team
will determine the final decision.
Participants will:
- Abide by the admissions policies of Milton ADS, which
include a history/physical and mantoux test or clear chest x-ray.
- Be willing to attend and participate in 50% of the
offered activities and programming.
- Benefit from the social environment as evidenced by
interactions with others.
- Feed themselves or eat independently with minimal
cueing.
Other issues to note
for admission:
Incontinence:
- Participant may not have uncontrolled or excessive
incontinence of urine or bowel as evidenced by accidents occurring
daily.
- Must display adequate hygiene practices or care from
home and must be free from odors and perfume. Arrangement for
help with personal care is available.
Communication:
- Must be able to communicate needs and/or follow simple
directions.
- Must be able to make their needs understood either by
speaking or adaptive equipment.
- Those who speak a foreign language will have an
interpreter available at all times.
Ambulation:
- Must be ambulatory with the assistance of cane, walker
or wheelchair.
- Must be able to stand and bear one's weight, pivot
and/or transfer from wheelchair or another chair and/or toilet with
assistance of one certified staff aide in a safe manner.
Wandering:
- May walk outdoors if attended and directable.
- Free to wander in Milton ADS halls alone or outdoors
if accompanied by staff.
Discharge Policy:
Policy: The following is to provide for the fair
and informed discharge from Milton Adult Day Services (MADS):
It is our intent to ensure an orderly process and to set
forth the requirements for consistent and appropriate dismissal of
Participants.
Those who are not eligible for admission include those persons whom
require more assistance than the MADS staff can provide, are beyond the
scope of services, those who have a contagious disease and those who
are a danger to themselves or others.
Due to the congregate nature of
the program, MADS reserves the right to refuse services
All MADS Participants will be monitored on a weekly basis to determine
whether or not they meet any of the criteria for discharge.
This will include but not
limited to the following:
General:
- Participant does not want to attend
- Requires 1-1 staff ratio of care
- Requires nursing care not provided by the facility
- Exhibits excessive excretion of body fluids, example
drooling
- When the mental capacity of the Participant interferes
with the staff's ability to assist with Activities of Daily Living
- Unable to eat independently
- Requires diet facility cannot accommodate
- Requires hospitalization, extended care facility or
hospice
Ambulation:
- Participants who cannot pivot for transfers, need
lifting or two-person transfer and those who cannot bear weight.
Incontinence:
- Uncontrolled and excessive incontinence which has not
benefited from two bathroom reminders/cues or as evidenced by two
changes of clothing within a daily visit
- Refuses assistance with incontinence or refuses
changes of clothing/depends
Program Participation:
- Sleeps excessively or isolates self, does not
participate in program activities 50% or more of the time
- Hearing or vision loss interferes with Participant's
benefits of attending
Wandering:
- Continual attempts to elope or leave building
- Participant is non-redirectable in his/her wandering
to unsafe areas or refuses to remain within secured safe area
Communication:
- When staff is unable to understand Participant
- When Participant cannot communicate needs and/or
severe Aphasia
Behavior:
- Combative behavior such as hitting, grabbing,
spitting, etc. is grounds for immediate discharge
- Threatening toward others by gestures. physically or
verbally (can result in a two-week discharge notice)
- A deteriorating physical/mental condition resulting in
Participant being a danger to self or others
- Inappropriate language such as swearing, cursing,
insulting or berating others
- Sexual comments and gestures/conduct
- Frequency and intensity or duration of Participant's
behavior continues after being monitored, care planned, and staff
intervention of the behavior remains uncontrollable and non-directable
- Disruptive behavior that affects the health, happiness
and well being of others or effects the operation of the program
- Participant currently on a two-week trial basis and is
deemed to be inappropriate
