Financial Assistance

Financial assistance is available for those who qualify through the following institutions:

Link Age, a department of REAL Services, Inc. (our Area Agency on Aging), will assist in completing applications for financial assistance that will cover the cost of adult day services. Link Age also offers Long Term Planning & Options, Benefits Counseling, Referrals and Disability Specialists. Intake Coordinators will take basic information and complete a phone assessment for the following funding programs:

• CHOICE (Community Home Options to Institutionalized Care)
• Medicaid Waiver
• Title 3-E ($1000.00 One time Caregiver Respite Grant which provides funding for in home services and Adult Day Services)
  

It is important to get on the waiting list for these programs for the elderly and disabled.

The Alzheimer’s Services offers a $500 grant for those with Alzheimer’s disease or related disorders only. Please request an application for this funding assistance.

To be eligible for VA health care benefits, the participant must be a veteran and a patient at the South Bend or Ft. Wayne VA Outpatient Clinic.
